Front end sign in not working

Hi,

I’ve installed and set the theme.
I’ve created a login page with the “login template” assigned to it.
When I try to login from the front page header, “nothing” happens.
The current page reloads with “/?” appended to the url.
When I look at the source code, it shows:
<form name="LoginWithAjax_Form" id="LoginWithAjax_Form" action="http://www.mywebsite.com//?" method="post">

It looks like the header doesn’t find the template or that I missed the spot to define it.
