Adding new tags gives DB error

This old chestnut: “Could not insert term into the database” whenever I try to add a new tag. Adding categories works fine.

I’ve encountered this issue before but upon checking the database the IDs in the wp_terms, wp_termmeta and wp_term_taxonomy tables are already set to auto_increment (AI). Am I missing something?
