Cannot Install theme too

I am having the same problem with GAME ADDICT! I tried everything. One click install gives me the “Are you sure you want to do this? Please try again” error.
FTP upload gives me the message blow many times:

Warning: Cannot modify header information – headers already sent by ....

I am doing a fresh install on wordpress 3.9.2.
I am investigating the problem and I found a mix of line termination formats in the theme’s php files.
Some php files contain Mac/Unix type of line termination character: LF (\n).
Other files contain non-identified termination format: CR (\r).
Other files contain Windows termination: characters CRLF(\r\n).
And other files contain a mix of the previous line termination formats. This is probably driving my webserver crazy.
Could you please solve the issue and help me install the GAME ADDICT theme???
