Can't Submit Project

Hi, I have some problems.
1. I can’t submit project on my website, since updating my theme to 4.1.7.
Every time I try to submit a project and click “SAVE” or “PUBLISH”, it suddenly refresh the page. And all the details in form was gone. I try to submit again and it’s still not submitted. And when I checked the roles, it’s already contributor.
So, what is happening with that?? Please help!

2. Then I also realized there are 3 pages of submit project which are /submit-project/ ; /submit-project-2/ and /submit-project-3/ after updating the theme. Which one I should use to submit project??
