funding press theme customization

However I want some more customization as mentioned below:

1)  I want to add fund offline through admin panel for a project, we are getting some funds through cheque or cash in person. How can we add it?

2)  http://fundmydream.in/project/crystal-concept/?step=1&chosen_reward=585&amount=0  in this page we want to bring the text box (where the user can enter any amount) as option like Rs.250, Rs. 300 etc. with radio button

3) We want to customize the ‘thanks for helping out’ form as below

• want to add few fields like phone number, website etc,
• Also we want radio button like “Do you want rewards: O Yes       O  No
• I don’t want list my name in backers section:  [ ] Yes

Please advise to implement the same.
