Registration broken

My registration page is completely broken and I’m not sure how to fix it. Users can’t register. The fields don’t exist. When you click on the register button, it takes you to basically a blank page. It was working, but for some reason it’s now completely borked.

Also, when it was working and a user registered, it says that it’s sending an email to the user to “activate” their account, but through my testing, no email is ever sent.

Any ideas?
